New Hampshire's Medicaid Decision Months Away

July 31, 2013 -- Low-income residents of New Hampshire won't know for months whether their state will expand Medicaid coverage in time to meet the Affordable Care Act's Jan. 1, 2014, deadline. That's when almost all Americans must have health insurance or pay tax penalties.

The state Legislature has been sharply divided over the issue on party lines, with Republicans challenging expansion and Democrats favoring it.

At issue is whether New Hampshire should allow Medicaid coverage to become available to adults under age 65 who aren’t pregnant, aren’t already eligible, and who have incomes less than 138% of the federal poverty level. That’s $32,499 for a family of four in New Hampshire and almost all other states.

A state commission will report to the Legislature about Medicaid expansion on Oct. 15, The Boston Globe reports. The newspaper says the Legislature could meet in a special session to put the expansion into effect. Expanded coverage would add tens of thousands of poor adults to the Medicaid rolls in the state, according to the Globe.  

It would take time for the state to let New Hampshire residents know that coverage is available, and they'd have to sign up by Dec. 15 for coverage to start on Jan. 1, 2014.

A 2012 Supreme Court ruling allowed states to decide whether to accept or reject Medicaid expansion. In states that do not expand Medicaid, people who would have qualified under expansion will be exempt from the individual mandate to have insurance.

States can expand Medicaid eligibility at any time, so it could go into effect in 2014 or later.  But enrollment periods, when residents can sign up for coverage, are limited each year.

July 31, 2013 -- For the time being, South Dakota has rejected expansion of Medicaid as allowed under the Affordable Care Act.

While the issue remains under consideration, don't expect anything to happen soon.

"The state is wrestling with many issues, particularly the impact to the state budget on a long-term basis, whether there will be access for individuals to primary care in the rural areas of South Dakota, the federal deficit, and the moral issue that individuals with the lowest incomes will not have access to subsidies through the federal Exchange," says Deb Bowman, a senior adviser to Gov. Dennis Daugaard. "If Medicaid is expanded, the [start] date would be up to the governor and Legislature but likely would be no earlier than July 1, 2014."

In 2012, the Supreme Court ruled that expansion of Medicaid was up to the states.

If South Dakota does expand Medicaid, coverage will become available to adults under age 65 who aren’t pregnant, aren’t already eligible, and have incomes less than 138% of the federal poverty level. That’s $32,499 for a family of four in South Dakota and almost all other states.

A task force created to study the issue is expected to submit its report by Sept. 15. The group started meeting in May and has one more meeting in August.

"It is a difficult issue with strong sentiments on both sides of the issue," Bowman says. "The task force report will clearly outline the pros and cons for decision-makers in the state."

Almost all Americans will need to either have insurance by Jan. 1, 2014, or pay tax penalties. However, in states that rejected Medicaid expansion, people who would have qualified under it will be exempt from the mandate to have insurance.

July 31, 2013 -- Iowa is ready to expand eligibility for Medicaid so more poor residents can get coverage as of Jan. 1, 2014. But it's waiting for the federal government to approve a unique approach under the Affordable Care Act.

The state wants to use federal funds to let residents buy insurance through its new Marketplace, instead of being required to get insurance through Medicaid itself, says Tom Alger, a spokesman for the Iowa Insurance Division.

For now, the state is expecting a waiver that will allow it to pursue this approach. "We don't have it yet, and we're waiting," Alger says. "Enrollment starts on Oct. 1, so it would be helpful to know in advance of that time."

Jennifer Vermeer, Iowa’s Medicaid director, told The Des Moines Register last month that she was optimistic about the state's waiver request. She noted that a similar program in Arkansas was moving smoothly toward approval.

The Supreme Court last year ruled that the federal government must allow states to accept or reject Medicaid expansion under the Affordable Care Act. Most states have already decided whether or not to approve expansion, which covers adults under age 65 who aren’t pregnant, aren’t already eligible, and have incomes less than 138% of the federal poverty level. (That’s currently $32,499 for a family of four in Iowa.)

Iowa, however, is one of the last states to act. Gov. Terry Branstad signed the expansion into law on June 20.The Des Moines Register estimates that tens of thousands of Iowans will be affected by it.

The federal government says people in states that don’t expand Medicaid who would have qualified under the expansion will be exempt from the mandate to have insurance or pay a tax penalty.

How Many Extra Calories Add Up to Obesity for Kids?

Study finds overweight children consume more excess calories daily than previously thought

TUESDAY, July 30 (HealthDay News) -- Overweight kids may be consuming far more calories than their doctors or parents realize, a new study suggests.

The study, which is published in the July 30 online issue of The Lancet Diabetes & Endocrinology, updates the mathematical model doctors use to calculate the daily calorie needs of children and adolescents.

The new model tries to more accurately estimate the energy requirements for growing girls and boys. It also accounts for kids' higher metabolisms, relative to adults, and takes into account the drop in physical activity that happens with age as frenetic toddlers turn into sluggish teens. And last, study authors factor in the increased energy required to maintain a bigger body size with age.

In sum, the model predicts that it takes far more calories for children to gain weight than experts had realized.

For example, the old model estimates that for a girl who's a normal weight at age 5 to become 22 pounds overweight by the time she's 10, she'd need to eat around 40 extra calories a day -- the equivalent of the calories in a small apple.

The new model predicts that she'd actually need to eat far more than that -- about 400 extra calories a day, or the calories in a medium serving of fast-food french fries -- to get the same result.

That's one case, but the number of calories it takes to gain weight is slightly different for boys and girls at every age.

"It's a bit of a moving target," admitted study author Kevin Hall, a senior investigator at the U.S. National Institute of Diabetes and Digestive and Kidney Diseases. "The point of these examples is that the excess calorie consumption is much larger than most folks would have suggested in the past."

Using historical data collected by the U.S. Centers for Disease Control and Prevention, Hall and his co-authors calculated that children today are an average of 13 pounds heavier than kids were in the late 1970s, before the start of the obesity epidemic. To gain those extra pounds, kids have consumed about 200 more calories a day.

At different ages, and depending on the amount of weight a child has gained, the differences can be even more stark.

For example, the model estimates than an 11-year-old boy who is about 18 pounds overweight has eaten roughly 320 more calories a day than his healthy-weight peers. Meanwhile, a girl who is the same age and also 18 pounds overweight has taken in an extra 301 daily calories.

Hall said the new numbers give parents and doctors a road map for "how we got here" with overweight and obese kids, but they aren't exactly the way back to a normal weight. Kids who cut calories by the amount their currently overeating may stop gaining, for example, but they'd likely need to cut even more to shed their extra pounds.

Toxins produced by blue-green algae are hazardous to people and can be fatal in animals, particularly dogs, said Deon van der Merwe, associate professor of diagnostic medicine and pathobiology at Kansas State University.

"Essentially anywhere there is water, you can find blue-green algae," van der Merwe said in a university news release.

"If there is a lot of sunshine and if the weather is warm, that makes it easier for them to grow rapidly," said van der Merwe, who also manages the toxicology section of the Kansas State Veterinary Diagnostic Laboratory. "That's why blooms occur most frequently in the summer, especially in drought conditions. Under drought conditions, you typically have many cloudless days and more sunlight reaching water."

Animals exposed to toxins from blue-green algae can experience vomiting or diarrhea. High levels of exposure to the toxins can cause them to experience liver failure and may be deadly. Gastrointestinal symptoms and liver failure also can occur in people exposed to these toxins, but symptoms such as skin rashes, sneezing, coughing, eye irritation and runny nose are more common in humans.

"If you go to a lake or pond and the water is green, it is important to avoid physical contact with that water," van der Merwe said. "If people swim or ski behind a boat and they inhale spray from behind that boat, those can also be situations in which people can be exposed."

"If people suspect that their dog has been exposed and the dog starts to vomit, that is often the first sign you will see," van der Merwe said. "It is very important to get the dog to the veterinarian as soon as possible. It may be possible to do preventive treatment to stop the full development of the poisoning."

Pet owners also need to protect themselves by wearing protective gloves when handling animals that have been exposed to blue-green algae.

In addition, blue-green algae can affect other animals -- such as birds, deer, cattle, horses and other livestock -- that use open water, van der Merwe said.

At the supermarket, unfortunately, grocery shopping can often be very boring. It will therefore be advisable to decide in advance the time needed to do the shopping, making a list of foods to buy. Better not go shopping when you are hungry, and if you were to give in and then enter the store, it is best to stay away from the food aisles. Always buy fresh produce and choose lean meats and fish, stay away from cold cuts, cheese, boxed meals, and avoid stopping in front of the snack. Better yet, prefer organic food stores, as far as possible. If you do not have the option to choose from, better to opt for the "lesser of two evils" and buy frozen fruit and vegetables, as they can keep as long as most of their nutrients and do not have hardly ever addition of sodium.

Do not skip meals

If you skip breakfast, you may lose power mid-morning. Rather than skipping breakfast completely, you may want to divide it into two parts: eat a small breakfast very early in the morning, like an egg, a small portion of oatmeal, or a slice of whole wheat bread. At mid-morning to eat a small snack, such as raisins and 10-12 almonds. This breakfast uniform is a much better solution than to look for a bar where binge of coffee and candies.

Taming the sweet tooth

We must all learn to curb cravings for sweets with fruit and stay away from sugary snacks and pastries. These foods contain a lot of calories and you can be added to the unhealthy fats. If the fruit does not seem to be quite sweet, you can add a small sprinkling of sugar. Avoid sugary drinks and prefer herbal teas or water with ice and a slice of lemon or lime.

Rome was not built in a day

If you can not change your eating habits easily, do not despair. Just start implementing some of these ideas, even one at a time, gradually, with patience and without trying to change from one day to another. Any change to their eating habits will be one more step in the right direction.

Obesity has become a growing problem in our country. According to the Centers for Disease Control and Prevention, 17% (12.5 million) children and adolescents ages 2-19 years are obese. This number has tripled since 1980. Our children are not the only ones who are obese; about one third of the nation's adults are obese as well.

Add obesity to the rising cost of health care and you have a real epidemic on your hands. In fact in 2003 former Surgeon General Dr. Richard Carmona said, "Obesity represents a much greater risk to Americans than any chemical or biological weapon that Saddam Hussein might be hiding underneath one of his sprawling compounds". While Saddam Hussein is no longer a threat, obesity still lingers, and much of the country faces this killer every day.

Now is the time to rise up against obesity and make America healthy again. As an ISSA Certified Fitness Trainer I've dedicated my life to doing just that. Many of us may be struggling with our weight, or we may have a child that may be struggling to find a healthy weight. Now is the time to change our habits and make healthy decisions for the future. I can remember growing up and never wanting to come inside, and even finding ways to be active indoors. As I look around today I see children and adults that have lost the drive to be active. Whether it is video games, television, computers or just your basic couch potato, we are now seeing an age that has lost the will for healthy survival. Verus is a story of overcoming the odds and the sheer will to survive which is at the very core of human nature. We need to find that will to survive for ourselves again, and gain back our freedom from obesity. So now is the time to turn off those televisions, video games, and computers. Go outside and play with your kids or pets, or simply go for a walk with a friend. The point is getting active. A Healthy Addiction

Most people would want to have a fit body. But, let's admit, not all of us would ruthlessly follow all the regimes set for it. It takes discipline, patience and perseverance to live, and continue living a healthy lifestyle. So then, how do healthy people keep themselves fit amidst all the temptations (such as food, carbs, carbs, carbs and fat) that surround them?

Here's a shocker. Fit people are addicted... to healthy living.

But what does healthy living mean? Here are some habits one should develop to be truly and totally fit.
Have a focus or an objective. A healthy lifestyle begins with a goal. What is our goal? How much weight would you want to lose? What exercise regimen would you wish to follow? What food would you stop eating? Set a goal and remember your goal. It will get you through your training and diet. Begin with an end in mind. Keep a positive mindset. Constantly tell yourself that you can do it, and, eventually, you will.

Sleep 6 to 8 hours a day. Living healthy means you have enough energy to go through with your day. Sleep replenishes energy that our bodies badly need. Aside from that, sleep deprivation can cause an unhealthy craving for sweets and can alter the way you eat. Oversleeping causes you to sleaze off for the rest of the day, too. So make sure you get just enough sleep. Not too little, not too much.

Drink lots of water. Drink at least 8 glasses a day. You don't have to down it all at once, of course. Drink a glass of water before going to bed, another after taking a bath, and if you wish to lessen food intake, drink 2 glasses before your meal. Water lessens your appetite and gives you that 'full' feeling. Drink more, especially if you sweat a lot and are exposed to a very humid environment. If you get thirsty, drink water instead of drinking sugary drinks (soft drinks, sugar-loaded juices, pop, etc.).

Less sugar, more veggies. Now, vegetables aren't always welcome friends. Most of us would shudder at the thought of those greens (and yellows, and reds, and... well, you get the point), yet, these are necessary for a balanced diet. Remember the food pyramid? Yup. That kind of balanced diet. Vegetables are easier to digest, promotes a healthy metabolism, plus, it can give you a better complexion. Some people would prefer to be vegetarians, or even vegan, but not all body types are capable of vegan/ vegetarian diets due to the protein needs one may have. Before you decide (and commit) to being vegan, might as well consult your dietician first. Sugary and fatty kinds of food should be avoided, so might as well cut back on the oil and butter. Junk food and soft drinks are a big no-no... along with food with a truckload of MSG and preservatives. Try to eat fresh to feel fresh.

Exercise exercising. See those runners/ joggers in the park? Try being one of them. Morning strolls are good, even just by walking your dog, you can get to move. That's the whole point of exercise... movement. Now, people would reason out that they don't have the 'time for exercise'. The thing is, it's not really much on the length of the exercise, it's about the repetition. Try doing an exercise regimen even just for 10 minutes a day. The most important thing is that you'll get to do it every day. Don't be contented on a one-shot weight loss treatment. As earlier stated, it needs discipline and perseverance to get the body and the lifestyle that you'd want.

Normalize your eating schedule. To promote a healthy digestion, eat regularly. When we say 'eat regularly', there should be proper spacing in between meals. More importantly, DON'T skip meals. Skipping meals makes you want to make up for the meal you skipped, you become hungrier and binge eat instead of eating normally.

Meditate. Take some time to empty your mind and release your stress. Stress is one of the causes of unhealthy lifestyle. It can cause lack of sleep, which in turn would result to unhealthy eating habits and sugar-craving.

Chronic depression affects every part of daily life, including sex. It curbs sex drive, yet sex can boost your mood and is important for relationships. And some depression medicines can curb your libido.

Breaking this cycle can be hard.

How to get out of this funk? There's no one-size-fits-all approach. But there are some tried-and-true ways to successfully treat depression without ruining your sex life.

What’s most important, experts say, is to never stop depression treatment out of fear that your relationships and sex live will suffer. That’s because depression itself can hurt relationships and may cause loved ones to take these problems personally.

“In depression, the chemical soup often isn’t right,” says Russell Stambaugh, PhD, a sex therapist in Ann Arbor, Mich., and a spokesman for the American Association of Sexuality Educators, Counselors, and Therapists. “The brain is important for sexuality because of the chemistry, but it’s also important for ideas. It helps how you experience pleasure and how you define it.”

Treat the Depression First

Whatever is happening with your sex life, it's important to treat depression first. Address any sexual side effects later.

Depression is the top cause of disability in the U.S. for people aged 15-44. Men and women struggle equally with sexual problems during depression.

People with chronic depression can experience a loss of desire, take longer to orgasm, and simply find sex less enjoyable.

“The whole process of sexual arousal starts with the ability to anticipate pleasure, which is lost with depression,” says Frederick K. Goodwin, MD, who serves on the scientific council for the National Alliance for Research on Schizophrenia and Depression. “People who are depressed are locked in the moment of their suffering.”

Drugs that treat depression, Goodwin says, “can release sexual function from the grips of depression.” Still, many antidepressants can affect sex drive.

“Medications can change sexual function,” says J. Michael Bostwick, MD, a psychiatry professor at the Mayo Clinic in Rochester, Minn. “But it’s a mistake for patients or physicians to immediately assume a difference in sexual function is bad. It’s different, and then you have to figure out if it’s bad or good. In fact, once the depression is treated, they may have a different view about sex altogether.”

Work With Your Doctor

Antidepressant drug side effects can be tied to the dose prescribed. So sometimes simply lowering the dose will treat the depression without blocking sexual desire, says Goodwin.

But don't tweak the dose yourself. Talk with your doctor if you have sexual side effects from antidepressants (or any other drug).

Goodwin says patients often don't start enjoying sex more until after being on an antidepressant for a few months. And there are antidepressants that don't affect sex drive.

TUESDAY, July 30 (HealthDay News) -- Certain diabetes blood sugar testing strips are being recalled by their maker, Nova Diabetes Care, because they may give users false, abnormally high readings, according to the U.S. Food and Drug Administration.

Twenty-one lots of the Nova Max Glucose Test Strips, as well as Nova Max Plus glucose meter kits that contain the test strips from the recalled lots, are part of the voluntary recall.

One expert called the recall "concerning."

"This is especially worrisome for patients who use insulin, as they depend on their fasting blood sugars to determine the amount of insulin they require," explained Dr. Alyson Myers, an endocrinologist at North Shore University Hospital in Manhasset, N.Y. "As a result, patients may over-correct their blood sugars, causing hypoglycemia [low blood sugar], which in severe cases can lead to loss of consciousness or permanent neurological damage."

"The symptoms of low blood glucose include shaking, sweating and confusion," added another expert, Virginia Peragallo-Dittko, executive director of the Diabetes and Obesity Institute at Winthrop-University Hospital in Mineola, NY. "People can pass out or have seizures if low blood glucose is not treated with glucose or sweet drinks right away."

Nova Diabetes Care said it has notified all registered users about the recall, along with health care providers who recommend the products, pharmacies that sell them and companies that distribute them.

Patients should immediately stop using the test strips from the recalled lots, the FDA said. They can check to confirm if they have test strips from the recalled lots by going to Nova Diabetes Care's website or by phoning the company at 1-800-681-7390.

Diabetes patients should be aware of symptoms of high blood sugar (hyperglycemia) and low blood sugar and should contact their health care provider for advice on how to treat the symptoms, the FDA said.

TUESDAY, July 30 (HealthDay News) -- A panel of experts commissioned by the U.S. National Cancer Institute says that the word "cancer" may need to be redefined to prevent overdiagnosis and overtreatment of conditions that are often not lethal.

Writing in the July 29 online edition of the Journal of the American Medical Association, the experts say that widespread cancer screening programs turn up too many growths that would not progress to a lethal stage and are considered "indolent."

Most patients do not understand that distinction, however, and "the word 'cancer' often invokes the specter of an inexorably lethal process," wrote Dr. Laura Esserman of the University of California, San Francisco, Dr. Ian Thompson Jr. of the University of Texas Health Science Center at San Antonio, and Dr. Brian Reid of the Fred Hutchinson Cancer Research Institute in Seattle.

Cancer can take "multiple pathways," the three say, "not all of which progress to metastases and death, and include indolent disease that causes no harm during the patient's lifetime."

This re-examination of what constitutes a cancer diagnosis has been spurred by the explosion over the past few decades of sophisticated screening measures such as the mammogram, colonoscopy and the PSA blood test (for prostate cancer). All were heralded as potentially lifesaving, and many predicted that widespread adoption of these tests would catch disease early and cause cancer rates to fall.

But the cancer screening story has turned out differently, the panel noted, because many of the lesions picked up on screening have turned out to be indolent.

"Screening for breast cancer and prostate cancer appears to detect more cancers that are potentially clinically insignificant," the experts said. The same might be said for screens for thyroid cancers and melanomas -- certainly, lives have been saved because tumors were detected and treated, but "the detection of indolent disease" has risen, too, the panelists wrote.

Issues like this have played out in recent years. The United States Preventive Services Task Force, an influential government panel, caused a furor in 2009 when it called for the abandonment of regular mammography screening for women under 50, reasoning that the benefits of screening for younger women were outweighed by the risks. The same panel also rejected the widespread use of the PSA test, noting that it too often picked up slow-growing lesions that might never harm men's health.

According to the experts writing in JAMA, the best-case scenario for cancer screening is when the tumor is slow-growing but also typically progressive. Colon cancer often acts in this way, the team noted, and colonoscopy has become an "effective" screening program.

In the meantime, however, "overdiagnosis" occurs. The experts say a redefinition of cancer may be needed to quell patient fears over indolent lesions and curb overdiagnosis and overtreatment.

MONDAY, July 29 (HealthDay News) -- A highly influential government panel of experts is recommending that older smokers at high risk of lung cancer receive annual low-dose CT scans to help detect and possibly prevent the spread of the fatal disease.

The The U.S. Preventive Services Task Force (USPSTF) concluded that the benefits to a very specific segment of smokers outweigh the risks involved in receiving the annual scans, said co-vice chair Dr. Michael LeFevre, a distinguished professor of family medicine at the University of Missouri.

Specifically, the task force recommended annual low-dose CT scans for current and former smokers aged 55 to 80 with at least a 30 "pack-year" history of smoking who have had a cigarette sometime within the last 15 years. The person also should be generally healthy and a good candidate for surgery should cancer be found, LeFevre said.

About 20,000 of the United States' nearly 160,000 annual lung cancer deaths could be prevented if doctors follow these screening guidelines, LeFevre said. Lung cancer found in its earliest stage is 80 percent curable, usually by surgical removal of the tumor.

"That's a lot of people, and we feel it's worth it, but there will still be a lot more people dying from lung cancer," he said. "That's why the most important way to prevent lung cancer will continue to be to convince smokers to quit."

Pack years are determined by multiplying the number of packs smoked daily by the number of years a person has smoked. For example, a person who has smoked two packs a day for 15 years has 30 pack years, as has a person who has smoked a pack a day for 30 years.

The USPSTF issued its draft recommendation Monday after a thorough review of previous research, and will take public comments on it until Aug. 26. A report summary will be published online July 30 in the Annals of Internal Medicine.

"I think they did a very good analysis of looking at the pros and cons, the harms and benefits," said Dr. Albert Rizzo, immediate past chair of the national board of directors of the American Lung Association. "They looked at a balance of where we can get the best bang for our buck."

The USPSTF is an independent volunteer panel of national health experts who issue evidence-based recommendations on clinical services intended to detect and prevent illness.

The task force has previously ruled on mammography, PSA testing and other types of screening. It reports to the U.S. Congress every year and its recommendations often serve as a basis for federal health care policy. Insurance companies often follow USPSTF recommendations as well.

Weighing heavily in the task force's latest decision were the results from the National Cancer Institute's 2011 National Lung Screening Trial. That study, which involved more than 53,000 smokers across the United States, found that annual low-dose CT screenings could prevent one of five lung cancer deaths.

People who drank more than four cups of coffee a day were 53 percent less likely to commit suicide than those who drank less than one cup a day, a new study found.

Those who drank two to three cups of coffee a day had a 45 percent lower risk of suicide, according to the analysis of data from more than 208,000 people who were followed from 1988 to 2008. During that time, there were 277 suicides, CBS News reported.

The study was published in the July issue of the World Journal of Biological Psychiatry.

The researchers said that caffeine in coffee can increased neurotransmitters, which can lift a person's mood and act as a mild antidepressant, CBS News reported.

"Unlike previous investigations, we were able to assess association of consumption of caffeinated and non-caffeinated beverages, and we identify caffeine as the most likely candidate of any putative protective effect of coffee," study author Michel Lucas, a research fellow in the department of nutrition at Harvard School of Public Health, said in a news release.
